How to report an incident?

Please reach out and create a ticket through the in-app chat by clicking the '?' icon at the top right corner of the screen and answer the following questions to help us better understand the situation.

  • Your full name.

  • What business was this with?

  • When did this happen (what was the date/ shift)?

  • What happened?

  • Did you get the names of any other people involved (if applicable)?

  • Anything else you would like noted?

Once this information has been provided we will pass on the information to your area manager who will reach back out to you for any next steps.
